Disclosure and Consent for Health Care and Therapy:

I fully understand that this treatment is not a substitute for medical treatment and that it may take several sessions before I notice any benefit. This will depend on my lifestyle, ongoing medication, and general health. I understand that the practitioners do not claim to cure or diagnose any medical condition in the same way as a doctor/physician. Their opinion is that of a holistic, complementary, and alternative therapist, and their professional opinions, advice, examinations, and recommendations do not constitute the medical advice of a doctor/physician. 

I understand that: 

• Linda Wright, retired medical doctor, and Joseph Garner, Master Herbalist, are not licensed physicians.
• The approaches used are intended to complement and not to replace conventional medical investigations and treatments.
• The treatment is alternative or complementary to healing arts services licensed by the state of Arizona.
• The services to be provided are not licensed by the state of Arizona.
• The services to be provided may include suggestions for diet, lifestyle changes, nutrition, and herbs.

Education, training, experience, and other qualifications regarding services to be provided:

• Linda Wright, MD (retired), has a BA in chemistry, achieved a medical degree at University of Colorado Health Sciences Center, completed a 3-year residency in internal medicine, and also completed a wide variety of alternative medicine education. She has experience working with clients for more than 35 years.
• Joseph Garner, Master Herbalist, completed a 3-year training program in Traditional Chinese Medicine at Colorado School of Traditional Chinese Medicine, graduated top of his class with a certificate in TCM, has worked with herbal medicine for more than 25 years, including teaching at Traditional Chinese Medicine schools for more than 8 years.

Insurance: Payment is expected at the time of service. Insurance does not pay for our services.
